Who is it aimed at?

Mandatory Training for social work staff based in the (0-25) Disability Team

What are the objectives?

The Codes of Practice to the Social Services and Well-being (Wales) Act lay down guidance for undertaking assessments and producing care and support plans under the Act. This training aims to develop participants’ knowledge in relation to:

  • undertaking outcome focused assessments.
  • producing care and support plans which accurately reflect the identified outcomes.

How will it make a difference?

It will support effective practice which reflects the requirements of legislation and statutory guidance in relation to undertaking outcome focused assessments and producing relevant care and support plans.

How to apply:

If you work for Carmarthenshire County Council (CCC) and are logged into a CCC device – PC, laptop, tablet or smartphone, complete this application form.

If you work for the Council but do NOT have access to a CCC device or an email address your line manager will need to apply on your behalf.

Council employees cannot apply for any courses from their own personal device.


Partners, agency staff or people who do not work for the Council cannot apply for this course.
