All reports must be led by a Direct Report to the relevant Head of Service (HoS). While other team members may contribute to drafting or supporting the report, the named Direct Report is accountable for the report throughout the process.
The report author is responsible for:
• Submitting the FWP request.
• Being recorded as the report author.
• Managing the report through the approval process.
• Meeting all deadlines.
• Providing the report to the HoS and Director within the required timescales for review and approval.
A Direct Report to a HoS can request a report to be added by submitting X Form[ link to the form here] to Laura Burt LJBurt@carmarthenshire.gov.uk
When submitting a request to add a report to the FWP, the relevant Head of Service must be copied into the initial email or submission.
Requests will not be accepted without HoS visibility and agreement on ownership and timescales.
Yes. All report cover sheets must be available in both Welsh and English before the report reaches the HoS Read and Approval stage for CMT. This is a mandatory requirement for all PIED reports.

Staff are encouraged to save the page as a favourite to ensure they are always using the most current documents.
Column M shows the date the Head of Service will review and approve the report.
Column N shows the date by which the Director must receive the final report.
Report authors should remain available throughout the approval process. Leave should not be booked on:
• HoS approval dates.
• The period between HoS and Director approval dates.
• Committee or meeting dates where the report is being considered.
This ensures authors are available to respond quickly to any queries, requested amendments or challenges.
Reports, together with both Welsh and English cover sheets, must be submitted at least 24 hours before the HoS review date and 24 hours before the Director review date shown in the FWP.
Reports may require amendments, clarification or additional information at short notice. The report author is responsible for ensuring these requests are addressed promptly to avoid delays to the approval process.
